Illness and UAs: *If you have been been exposed to or have symptoms of any contagious disease, we encourage you to obtain medical attention as soon as possible. However, simply telling your monitor you have been exposed to an illness will not excuse you from a requested UA. Please take all feasible precautions to avoid a potential spread of illness as you submit to the UA request. *Please note; You will need documentation from a medical professional for you to be excused from a UA request. *Please email documentation (paperwork you received from the doctor or clinic) to email@example.com. You will then need to follow-up your email with a call to your monitor, Brian Foxworthy, at 913-390-0100, for confirmation as to whether or not you are excused from a requested drug/alcohol UA.
